Mandatory Minimum Requirements To Apply:

You must meet the following minimum requirements by the application deadline:

• U.S. citizen
• Must have a valid driver's license
• Must be 21 years of age and under 35 years of age at the time of application, unless otherwise exempt by statute
• No felony or misdemeanor convictions as specified in Chapter 65 ILCS 5/10-2-1-6
• Must be an Illinois State certified paramedic at time of hire
• Must have a current CPAT card valid within 12 months of test date
• Application must be submitted with all required information 

Further Mandatory Requirements:

• Mandatory Orientation
• Written Examination
• A current CPAT card with ladder certification valid within 12 months of test date
• Oral interview After successful completion of the above, applicant must pass:
• Background Check
• Polygraph Examination
• Psychological Assessment
• Drug and Medical Examination 

Upon appointment, applicant must successfully, attend and graduate from Fire Academy and complete a one (1) year probationary period.

• Firefighter/Paramedics requirements (applicant to retain)
• Test dates and locations fact sheet (applicant to retain)
• Rules and regulations agreement/test result waiver (complete and submit)
• Waiver/release of liability (complete and submit)
• Background investigation agreement (complete and submit)
• Firefighter Application (complete and submit) 

In addition, you MUST include a legible copy (no originals) of the following documents with the completed application forms listed above:

• High school diploma or GED equivalency
• Paramedic certification (if certified or prior to time of hire)
• Verification of CPAT with ladder certification valid within 12 months of test date
• Birth certificate
• Current valid driver's license (in color)
• Recent photograph, maximum size 2"x 2", minimum l"x1" (passport photo, etc.)
• Military DD214 (if applicable) 

Must be in good moral character as determined by the Calumet City Board of Fire and Police Commissioners.

Testing may be conducted by Calumet City Fire & Police Commission and C.O.P.S. & F.I.R.E. Testing Service, Inc.

• Any pending paramedic certification must be provided to the Board immediately upon completion.
