Minimum Requirements:

Required documentation due 11/1/2024 at 2:00 PM:

• Consent and release form (2 pages) – available after download
• Copy of high school diploma
• Copy of valid driver’s license
• Copy of birth record as proof of citizenship status: can include: county/state-issued birth certificate (hospital issued birth certificates not accepted), U.S. passport, naturalization papers
• Copy of valid IDPH Paramedic license or proof of enrollment in paramedic program
• Copy of valid CPAT dated 11/1/2023 – 11/1/2024

Testing Information:

The written examination will be given in a virtual format, and you will take the exam in your own home. The virtual exam will be scheduled during business hours the week of December 2nd – no weeknight or weekend sessions will be offered. You are responsible in ensuring you have the flexibility to test during this time. Additional details and instructions will be provided to eligible candidates.
